Covid-19 Vaccinations
Covid-19 (Coronavirus) vaccines are now available at Whole Family Medical Care.
You do not need to be a Whole Family member patient to be vaccinated at our office. Member patients will receive Covid-19 vaccinations at no charge. For others, there will be a $10 fee. You do not need to have medical insurance to be vaccinated.
APPOINTMENTS ARE NEEDED FOR VACCINATIONS.
PLEASE DO NOT COME TO OUR OFFICE FOR A VACCINATION WITHOUT A CONFIRMED APPOINTMENT. WE WILL BE FORCED TO TURN YOU AWAY.
To schedule your vaccination:
STEP 2
Download and complete the secondary registration form. It's available as PDF in English and Spanish. You must bring the completed form with you when you visit our office to be vaccinated. You may also open the form in Adobe Acrobat, click on the form's fields and check boxes and type your answers. Type your name for a signature. Then save the PDF and email the form to: Wholefamily@myupdox.com
STEP 3
Use our online scheduler. If you have any difficulty with the online scheduling system, call 419-872-3250
STEP 4
Wait to be contacted by our office. We will call or text you to confirm the closest appointment time available to your request. Do not come to our office without confirmation of your requested appointment time.
